Shrinked posterior pole of a detached retina is a sign of epiretinal gliosis. Another sign is the vessel's tortuosity. Reattachment of such a retina may be possible, but will cause to the patient very disturbing metamorphopsias postoperatively.
Removal of the complex ILM + mERM will give to the patient a more satisfactory postoperative vision without metamorphopsias...

The surgery of this condition demands the removal of the ILM from the posterior pole around the macula (between the arcades). ICG is a beautiful dye to make this membrane visible but is considered to be toxic for the pigment epithelial cells of the macula (foramen) and the exposed photoreceptors in the detached retina around the hole. So we have to dye but we must avoid this contact.
